The population in Hong Kong is ageing, and the number of older people with chronic conditions will continue to grow. The Hong Kong Jockey Club Charities Trust has adopted a proactive approach to strengthen the support for senior citizens, and launched the “Jockey Club Community eHealth Care Project” (“the Project”) in 2016.
The Project is the first territory-wide elderly care project to integrate digital health management technology, community care and professional support. The aim is to support chronic disease management and the healthy ageing of older people. Elderly centres are being developed as a first point of contact, with digital technology utilised to raise health awareness and for self-management. Data derived from the project will be utilised for broader data analytics.
The Project entered its Final Phase in 2023, expanding its coverage to 157 elderly centres across 18 districts in Hong Kong, and benefitting more elderly to better manage their chronic conditions and improve quality of life.

Project Objectives
- To apply digital health technology to empower older people to build self-management habits
- To promote elderly centres as the first point of contact for detecting and addressing the health and social needs of the elderly for healthy ageing
- To encourage stronger linkage between elderly centres and community health services
- To collect, aggregate and analyse the health characteristics and patterns of the elderly through data analytics
