Well-being survey
It aims to detect health and social needs of elderly, allowing them to better understand their health and for elderly centres to provide targeted and evidence-based health promotion programmes.
eHealth station
Elderly are encouraged to monitor their blood pressure, blood glucose levels and other health measurement data at the eHealth stations in the elderly centres or at home for self-management. They are also empowered to upload their measurement activities and data to the Government’s Electronic Health Record Sharing System (eHRSS).
Digital portal
Health data from the eHealth stations are transferred to the digital portal by cloud technology for monitoring and data analysis.
Health promotion programme
Elderly identified with health needs can enroll in health programmes on sarcopenia, nutrition or psychosocial support, etc., at the centres.
Health referral
To address older adults’ health needs, elderly centre staff will provide referrals to health care services in the community. It enhances linkage with existing health services.
