Positive impact of self-management

Through regular measurements at eHealth stations, more than one-third of participants have either systolic blood pressure (SBP) dropped by ≥10 mmHg or diastolic blood pressure (DBP) dropped by ≥5 mmHg, lowering the risks of coronary heart disease and stroke.

ICOPE implementation readiness

Surveys1 were conducted with NGO partners to assess their readiness to implement Integrated care for older people (ICOPE) in their centre.

Service implementation:

Overall, the percentage of NGOs reporting sustained service-level actions increased at follow-up compared to baseline.

System implementation:

Overall, the percentage of NGOs reported sustained system-level actions increased at follow-up compared to baseline.

Note 1: 42 staff from collaborating NGOs were invited to complete an online questionnaire. Pre-test was conducted in November 2023 with post-test conducted in February 2025. The survey includes 19 implementation actions, with 9 actions at the service level and 10 actions at the system level. Staff can assess the extent to which each action has been implemented at the elderly centre by rating it in three stages: "None to Minimal," “Initiating," and “Sustaining."

Note 2: The maximum total implementation score is 52 points. A total score 0-20 points indicates that the centre is at the "None to Minimal" stage in implementing Integrated Care for Older People (ICOPE); 21-37 points indicates the "Initiating" stage; 38-52 points indicates the "Sustaining" stage.
